Benefit: From addressing everyday health questions to offering advice on managing serious health conditions, WH365’s Patient Navigators guarantee access to trustworthy health information and services, offering the prompt support you need from a trusted source.

QUICK ACCESS TO GENERAL PRACTITIONERS & LICENSED MEDICAL SPECIALISTS

WH365 Service: This feature provides members with the flexibility of VIRTUAL and SCHEDULED, IN-PERSONappointments with General Practitioners (GPs), ensuring convenience and accessibility.

Similarly, Patient Navigators (PNs) also facilitate quick scheduling with Specialists.

Benefit: This approach ensures convenience and easy access to medical care, significantly reducing reliance on emergency departments for primary healthcare needs. By facilitating quick scheduling with healthcare professionals, the service promotes early diagnosis and treatment, leading to potentially better health outcomes.

This integrated healthcare model not only makes medical care more accessible but also aims to improve the efficiency and effectiveness of the healthcare journey for each member.

QUICK ACCESS TO MENTAL WELLNESS CARE

WH365 Service: For WH365 members, this service focuses on prioritizing Mental Health by providing expedited access to OHIP covered Psychiatrists. Members referred to Psychiatrists will be assessed within two weeks of their referral being processed.

Dedicated to cultural competence and racial sensitivity, WH365 tailors its care to honor the distinct background, culture, and experiences of each person, guaranteeing respectful and customized treatment.

Benefit: Quick access to Psychiatric Evaluations can lead to earlier detection and diagnosis of mental health issues, improving the chances of successful treatment.

Knowing that mental health support is readily available, and prompt, can significantly reduce the anxiety and stress associated with long wait times for psychiatric care.

QUICK ACCESS TO DIAGNOSTIC IMAGING & CONCIERGE PHARMACY SERVICES

WH365 Diagnostic Imaging Service: This service is uniquely structured by WH365 to dramatically shorten the usual waiting periods for imaging tests such as MRIs, CT scans, X-rays, and ultrasounds.

Leveraging WH365’s extensive network, which includes affiliations with hospitals and private diagnostic imaging centers across Ontario, the service capitalizes on available, last-minute appointment slots. This strategic approach enables the quick scheduling of WH365 members, ensuring they receive timely access to necessary imaging procedures.

WH365 Concierge Pharmacy Service: Our concierge pharmacy network offers FREE medication delivery services, bringing prescriptions directly to the members home or workplace to provide convenience for members who may have difficulty getting to the pharmacy.

WH365’s Concierge Pharmacy services also include patient education and counseling on medication use, potential side effects, interactions with other drugs or food and other aspects of disease management. This can help members become more informed and active participants in their care, leading to better health outcomes.

WH365 Service: WH365 provides members access to a Health Information & Navigation Gateway which delivers reliable health information, personalized support, and effective health-related services.

Benefit:  The portal contains a relevant, reliable, and up-to-date collection of articles, resources and search tools that offer insight into common health concerns and how to navigate our healthcare system, as well as databases for health conditions, medications, and natural products.

Our smart search tools let users quickly find physicians/specialists, hospitals, clinics and labs, community support groups, and caregiving facilities using a postal code or specific intersection.

Having prompt and engaging telephone support enhances our member’s experience. Our approach is to actively listen, reassure and support member’s needs with information, resources, and guidance.

With WH365’s Health Navigation telephone support, our members receive access to experienced Health Information Specialists through a toll-free number that helps them make informed health decisions for themselves and their loved ones.

LOCATE HEALTH SERVICES

Members can utilize our Intuitive location-based search tools to easily find physicians, specialists, hospitals, clinics, labs, community support groups, and caregiving facilities based on postal code or specific locations.

Our search tools provide a vast collection of current and reliable articles, resources, and tools, providing insights into health concerns and navigation of the healthcare system. It includes databases on health conditions, medications, and natural products.

RECEIVE CUSTOMIZED CASE REPORTS

Members can receive customized case reports containing additional information and resources, including lists of services, physicians, and health education materials, normally provided within one to two business days of the request.

These information requests are reviewed by our Nurse Clinical Coordinators and our Medical Director (physician) as required.

HEALTH RISK ASSESSMENT

WH365 offers a Health Risk Assessment Program for all members, featuring an easy-to-complete online questionnaire that identifies and targets modifiable health risks.

Through personalized reports and tailored action plans, members gain insights into their health habits and receive guidance on improving them.

Based on current clinical guidelines, this program empowers members to understand and address their health risks effectively, providing valuable information and resources for their well-being.

After completing the questionnaire, members will receive:

» A personalized report highlighting areas of strength and improvement in key risk areas

» Reliable information about their health habits

» Personalized action plans with tips on how to improve their habits

» Information about available public services and resources that are available to them

MEDICAL SECOND OPINION

When diagnosed with a critical illness, having a second medical opinion can make all the difference in treatment and recovery. For our members diagnosed with a critical illness or life-threatening condition, the Medical Second Opinion is a trusted and tangible service that is useful for our members.

The WH365 Medical Second Opinion program is a truly Canadian solution that helps fill the gap in health care many Canadians currently experience. Understanding the unique problems that Canadians face with managing their health and working with them alongside their primary caregivers results in better outcomes for your members and their families.

WH365’s in-house medical team and Medical Advisory Board work with expert physicians from academic medical centers across Canada, selecting the best and most qualified specialist to review and analyze each case. These specialists are internationally recognized for their: Experience (with rare and complex disease), Knowledge (of the latest medical research and treatment protocols) and Commitment (to evidence-based medicine).

Note. WH365’s Medical Second Opinion Feature is not included in the membership fee.   Members, however have access to this service and pricing is based on a per-case basis depending on the complexity of the case.
